Atenolol
Indications/Uses
HTN, CHD, tachyarrhythmias, hyperkinetic heart syndrome.
Dosage/Direction for Use
HTN Initially 50 mg in the morning, may be increased to 100 mg after 1 wk. CAD 50 or 100 mg once daily. Tachyarrhythmias 50 mg once daily or bd, or 100 mg once daily. Hyperkinetic heart syndrome 25 or 50 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. NYHA class III or IV heart failure; shock; 2nd- or 3rd-degree AV block; SA block; sick sinus syndrome; bradycardia (<50 bpm); hypotension; advanced stages of peripheral occlusive disease; chronic obstructive airways disease; metabolic acidosis.
Special Precautions
Patients w/ renal dysfunction. May mask signs of hypoglycemia. Discontinue therapy by slow downward titration in patients w/ CAD. Atenolol therapy in patients w/ pheochromocytoma must be accompanied by concurrent α-receptor blockade. Possible reduction of lacrimation. May impair ability to engage in activities requiring mental alertness. Pregnancy & lactation.
Adverse Reactions
Fatigue, dizziness, drowsiness, mild headache, sweating, cold extremities.
Drug Interactions
Additive effect on the sinus & AV nodes w/ Ca-antagonists of the verapamil or diltiazem type. Exaggerated BP increases w/ norepinephrine or MAOIs; abrupt w/drawal of concomitant clonidine. Exaggerated BP reductions w/ diuretics, phenothiazines, narcotics, or other antihypertensives, hypotensive agents. Additive cardiodepressant effects w/ narcotics or anti-arrhythmic drugs. Enhanced negative chronotropic & negative dromotropic effects w/ reserpine, clonidine, α-methyldopa, guanfacine, &/or cardiac glycosides. Enhanced neuromuscular blockade of tubocurarine. Induced hypoglycemia & masked symptoms w/ insulin or oral antidiabetics.
